Company Registration Copy	Indian company registration; if in another language, translated into English and notarised.

Valid Passport	Valid for at least 6 months (recommended) and issued within last 10 years.
Application Form & Photos	One application form (online/print) + two photos 35×45 mm, white background.
Original Work Permit	Polish employer’s work permit approval for the applicant.
Invitation/Employment Letter	From Polish company, addressed to Polish consulate, includes applicant’s details.
