| Company Income Tax Papers | For the last 3 financial years of Indian company. |
| Personal Bank Statement | Last 6 months, with bank stamp & signature on each page. |
| Company Bank Statement | Last 6 months, with bank stamp & signature on each page. |
| Salary Slips | Last 6 months for the applicant, stamped & signed. |
| Return Air-Ticket | Confirmed return or onward ticket. |
| Company Registration Copy | Indian company registration; if in another language, translated into English and notarised. |
| Travel Medical Insurance | Covering minimum €30,000 for entire stay. (Gov.pl) |

For Indian citizens visiting Poland for leisure, tourism or private visits.
